Madison Helms RN BSN is our school nurse. She is a magna cum laude graduate of the SUNY Plattsburgh Bachelors of Science Nursing Program. She is also a cum laude graduate of the Associates of Applied Science Nursing Program through SUNY Adirondack. She has worked in inpatient adolescent and pediatric mental health, outpatient primary care, as well as short term inpatient rehab as a nursing supervisor. Mrs. Helms performs the mandated health services for our students.

Health services at NCS include:

Evaluation and treatment of ill or injured students and staff as is possible at school and within the scope of practice for a Registered Nurse

Administration of medication as indicated and ordered by medical providers

Health education

Illness surveillance

Development and implementation of health care plans for students with chronic conditions

Communication with parents around health issues for their child

Development/revision of health policies as indicated

Ms. LaFond is also the school's attendance officer and issues working papers during the school year. During the summer working papers can be issued through the school business office by appointment. If you are applying for working papers please ensure you and your student have completed the required AT-17 form posted below, and bring a copy of a physical exam conducted by a NYS health care provider within the past 12 months.

Physical exams are required for all NEW students entering a school in NYS as well as Pre-K or Kindergarten, 1, 3, 5, 7, 9, 11. Per NYS, it is required to use the NYS Physical Exam Form posted below. Please also note the exam must be conducted by a NYS provider within the past 12 months. 

Your student must have had a physical exam completed within 12 months prior to starting any sports season. If your child's physical exam expires during the sports season, they may play through the entire sports season, even if the exam’s expiration is before the season is complete. However, an up to date physical will be needed for all other sport seasons that year. Prior to participation in each athletics sports season, all students will be given an interval athletic health history form which is attached below and is to be completed and signed by the parent/guardian.

In the event a student is unable to participate in gym activities due to illness, injury, etc., a parent/guardian may request this in writing to the school nurse for one day. Anything beyond one day requires a written note from your healthcare provider. Anytime a student is unable to participate in gym activities they are not allowed to participate in recess.

For your convenience, we have attached our school policy regarding illness for the 2021-2022 school year.

Mandated screenings consist of: 

Vision

Distance and near acuity for all newly entering students and students in Pre-K or Kindergarten, Grades 1, 3, 5, 7, and 11

Color perception screening for all newly entering students

 

Hearing

Hearing screening for all newly entering students and students in Pre-K or Kindergarten, Grades 1, 3, 5, 7, and 11

Scoliosis

Scoliosis (spinal curvature) screening for all girls in grades 5 and 7, and boys in grade 9

Please note that vision and hearing screenings will not be repeated in the school building if they were completed by your child's primary care provider outside of school. Screenings for new students will be conducted within 6 months of entering the school.
